Field notes into a typed report that separates what you saw from what you were told
What it does
/as:site-visit-report turns field notes, photos, files, or a conversation into a structured walkthrough record.
The discipline is in the typing. Five categories stay separate and stay labeled:
- Direct observation — what you saw
- Participant-reported information — what someone on site told you
- Interpretation — what you concluded from either
- Limitations — what you could not access or verify
- Proposed follow-ups — what should happen next
Conflating the first three is how a contractor’s offhand remark ends up cited months later as a measured condition. The report keeps them apart on the page.
What it does not do
Saving a report changes nothing else — PROJECT.md, decisions/, and TASKS.md stay as they were. You promote selected items afterward, to /as:project or /as:tasklist, and you choose which.
